Every fact on this site comes from a government source, and each one is shown with its source link and the date we last checked it. We do not collect reviews, estimate anything, or rank facilities. When an agency does not publish a value, we write “Not published.”
Nursing homes: CMS
Every nursing home page is generated by a script from CMS Provider Data Catalog files on data.cms.gov. Medicare Care Compare uses the same data. This build uses files CMS processed on September 1, 2026:
- Provider Information: ratings, staffing, inspection-cycle totals, fines summary, ownership type and chain
- Health Deficiencies and Fire Safety Deficiencies: every citation for the last three inspection cycles
- Penalties: fines and payment denials
- Ownership: owners and managing entities reported to CMS
- State and US Averages
- Definitions come from the CMS Nursing Home Data Dictionary and the Five-Star Technical Users’ Guide
Every Medicare- or Medicaid-certified nursing home in each launched state is included. Facility phone numbers are left out on purpose.
Assisted living: state agencies
- Louisiana: rules and lookup steps from Louisiana Department of Health, Health Standards Section (HSS); facility license list from the Licensed Providers Report 2026-10 (Adult Residential Care).
- Texas: rules and lookup steps from Texas Health and Human Services Commission (HHSC); facility license list from the Directory of Assisted Living Facility Providers with an Active License as of 2026-10-22.
- Florida: rules and lookup steps from Florida Agency for Health Care Administration (AHCA); no facility list (the state offers interactive tools, not a download).
- Georgia: rules and lookup steps from Georgia Department of Community Health, Healthcare Facility Regulation Division (HFR); no facility list (the state offers interactive tools, not a download).
Freshness
CMS refreshes its nursing home files monthly. State lists are refreshed when the agency posts a new file. Each page shows the data date and the last-checked date. Always confirm on the linked official record before you decide.
Corrections
Email toffee@mail.grokbot.com with the facility’s CCN or state license number. We fix errors by re-checking the official source, not by taking edits from facilities.
